Financial Statements

Please provide a copy of one of the following financial documents from within the last two fiscal years:

• For organizations with annual revenues of $750,000 or more: Independent financial audit.

• For organizations with annual revenues between $300,000 and $749,999: Independent financial statement OR an independent financial audit.

• For organizations with annual revenues under $300,000:  Internal financial statements (990s are fine). In addition, please list the following in the organization’s history section of the proposal:

• Conflict of Interest Disclosure: Please share any potential conflicts of interest, including family ties to applicants or financial connections with vendors.

• Active Legal Matters: Please also list any current or pending legal actions that could affect the organization or the proposed project.
